On June 16, 2021 we will host Community Living Mississauga’s 66th Annual General Meeting (the “Meeting”). This year’s Annual General Meeting will be virtual and held online via Zoom again this year due to safety issues related to COVID-19. The business meeting itself will not be followed by a community celebration and awards presentation BUT we have added some excitement to our meeting with a guest speaker! In order to attend the Meeting, Members are required to pre-register by RSVP to email@example.com. Pre-registered Members will receive instructions on how to access the virtual Meeting via email and the Meeting will commence at 6:00 p.m. (EDT).

- Proxy Form (Note: to be valid, the form must be signed. Please download the form, complete the appropriate sections, print it, sign it, scan it and email it to firstname.lastname@example.org by the June 4th deadline or follow directions for mailing or drop of by the deadline).
Members are encouraged to submit a proxy in advance in order to ensure a smooth and efficient meeting. Members who have completed a proxy form may still view the Meeting’s proceedings by following the instructions to be provided via email after pre-registration.
However, Members who do wish to participate and vote at the Meeting should follow the instructions set forth below under heading “To Vote at the Meeting”.
TO VOTE BY PROXY: Members are asked to complete a proxy form no later than 5:00 p.m. (EDT) on June 4, 2021 by email to email@example.com.
TO VOTE AT THE MEETING: To attend and vote at the Meeting, Members must be pre-registered and sign-in to the online Zoom meeting no later than 5:45 p.m. (EDT) on June 16th (at least 15 minutes prior to the commencement of the Meeting) by following the instructions to be provided via email after pre-registration.
